Your
First Appointment
Thank you for your interest in the ReproMed Fertility Center. We understand
the anxiety associated with meeting a new physician in a new practice.
We are here to alleviate your fears and support you during this process.
What to expect at the Initial Appointment
Your initial appointment will be scheduled promptly once you contact
us. Plan at least one hour for your first visit. Dr Pinto recommends
that you invite your partner, or whoever helps you make health care
decisions to join us at this meeting. You will meet with Dr. Pinto
to discuss about your medical history, any prior tests and treatments
and your diagnosis as well as future treatment options, if appropriate.
Following your initial evaluation, an ultrasound may be performed.
- Please bring copies of all medical records relating to infertility,
on yourself and your partner. You will need to fill out the "Request
for Medical records" form and send it to your physician before
your appointment.
- In order to maximize the benefit of your first meeting, you
are encouraged to address few important issues prior to your appointment.
Please take a moment to download some of the required forms below.
It is advisable to complete all paperwork prior to your appointment.
Alternatively, you have the option of doing this at the office.
- It is advisable that you contact your insurance carrier to determine
your insurance coverage prior to visiting our office. Please bring
your insurance card along with you.
